Task Force Screening Process

DANCE/USA ANNUAL POST-HIRE HEALTH CARE SCREEN FOR PROFESSIONAL DANCERS
SCREENING PROCESS 
2014-2015 SEASON

  1. Registration form MUST be completed before any screen documents are disseminated.  The form MUST be filled out every season, regardless if the form has been filled out previously.  In order to complete the registration form, the company manager and/or executive staff of the dance company will likely need to provide some of the information.  The registration form must be sent to BOTH Jenna Calo (jcalo@nullbodydynamicsinc.comAND Heather Herod Cole (heathercole@nullp3nashville.com).
  2. Once the registration form is completed and provided to Heather and Jenna (see above), the MOST UPDATED screening form & guidelines will be sent out via email.
    1. A video for conducting the screen is available on the Dancer Wellness Project (contact galbraith.gary@nullgmail.com).  Heather & Jenna may also be contacted with any questions.

      Each company decides how best to implement the screen, what to do with the information, how to store data, etc.  Recommendations for these items are listed in the guidelines.
  3. IF THE INFORMATION IS GOING TO BE USED FOR RESEARCH PURPOSES IN ANY WAY:
    1. Heather Southwick MUST HAVE the completed:
      1. Individual Investigator Agreement
      2. CITI certificate
    2. Please contact Heather Southwick directly  at hsouthwick@nullbostonballet.com
    3. The research protocol resides as an umbrella protocol under Boston Children’s Hospital.  In order to receive the INFORMED CONSENT FORM for your dancers and use the information you collect as research or data, you need to be CITI trained.  Boston Children’s Hospital’s IRB no longer accepts any other form of human subjects training.  NIH or CREC training is not acceptable.
    4. Once the required documents are completed and submitted to Heather Southwick, the INFORMED CONSENT FORM will be sent by Heather Southwick.
    5. If you have your own IRB, please be sure to share the protocol information with BOTH Heather Southwick AND Gary Galbraith in order to use the data for research.
  4. It is recommended that the Dancer Wellness Project be utilized for data storage.  Contact Gary Galbraith for information:  galbraith.gary@nullgmail.com.
